YOU WILL BE CALLED AND GIVEN AN ESTIMATE FOR THE REPAIRS YOUR VEHICLE NEEDS BEFORE ANY WORK IS PERFORMED, UNLESS OTHERWISE ARRANGED.

NOTICE: THIS FORM MUST BE SUBMITTED BEFORE WE CAN BEGIN WORK ON YOUR VEHICLE.

By submitting this form, you hereby grant TTL Automotive permission to operate the vehicle herein described on streets, highways or elsewhere for the purpose of testing and/or inspection. TTL Automotive will not perform repairs to your vehicle until you have provided authorization of any amount. You agree that TTL Automotive is not responsible for loss or damage to vehicle or articles left in vehicle in case of fire, theft, or any other cause beyond our control or for any delays caused by unavailability of parts or delays in parts shipments by our suppliers or transporters.
